There is no form to fill in before you can talk to someone. Call us, describe the situation however it comes out, and we will tell you on that first call whether we can help.

Who can refer
Anyone can
- Yourself, as a participant
- A family member or carer
- A support coordinator or LAC
- A hospital discharge or allied health team
- A school or transition worker
- Another provider handing over
You do not need
- A referral letter
- To know which supports you want
- To know your registration groups
- Your plan in front of you
- An email address
Working out which supports fit is our job. Turning up with nothing but the situation is completely fine.
What we will ask you
Not much, and none of it is a test. Roughly:
- What is happening right now, including the parts that are not working
- Where you are, so we can check whether we can staff it properly
- Roughly what your plan funds, if you know. If you do not, that is fine
- Who else is involved, such as a coordinator or family
- How you prefer to be contacted, and whether you want an interpreter
What happens after you call
Same call: we tell you whether we have capacity. Within 4 business hours if you emailed: a named person replies. Then: we come and meet you, at home or wherever suits, before anything is agreed.
If we are not the right provider, we will say so on that first call and point you toward someone who is. We would rather do that than put you on a list and go quiet.
Referring on behalf of someone else
If you are a support coordinator, LAC or another provider, our coordinators page has our scope, current capacity and how we report. Send what you have. If a service agreement or risk assessment already exists, that saves everyone a fortnight.
